Request a Lab Draw & Consultation

Creating your customized care plan begins with having your labs drawn at a convenient Quest Diagnostics location. You will receive a lab requisition form via email and may schedule a lab appointment at your local Quest at your convenience. Labs may be processed through insurance but the remainder of care will be processed as self-pay. HSA/FSA cards are accepted for treatment. 

Once your bloodwork has resulted, you will receive a link to complete your medical intake forms and schedule your consultation appointment. Appointments are virtual and take place over Google Meet. Medication is then shipped directly to your door if treatment is indicated.
